New Patient Registration

Anyone in the UK can register with us. However, only patients who live within the following areas and who meet the clinical criteria will be able to receive a home visit:

lNE21, NE39, NE40, NE16, NE17, NE41, NE42.

There are 2 ways to register at the practice:

1. The fastest way to register is to complete an online registration by clicking here

2. Attend in person and complete a registration form

Please note, If you are already registered as a patient at our Surgery, you do NOT need to register again to use our smartphone app or submit an econsultation / online consultation.
